The preschool program services children that turn 3 or 4 years of age on or before August 1st. Classes are located at Mt. Logan Elementary. Our preschool program is accredited by the National Association for the Education of Young Children Academy and licensed by the Ohio Department of Education. Registration in the program requires:
- Birth Certificate
- Social Security Card
- Up-to-date Immunization Record
- Physical Form completed by a physician and dated after July 1st.
- Proof of Income for the household
- Proof of Address
- Divorce/Custody documents(if applicable)
Forms that will need to be completed for registration are: emergency card, registration form, parental information, parent memorandum of understanding, income verification, early release, release of records, home visit, family needs assessment, lunch form, emergency medical authorization and confidential medical and family history.
